Summary:
The first form was completed within days of his having quit the job briefly held in January and I can only conclude in light of his failure to take care to ensure that the form was completed accurately, that his carelessness was tantamount to knowingly making a false statement.

Summary:
There is just cause for leaving employment after a brief trial when it becomes apparent that the duties to be performed are not those anticipated by the employee and are not clearly within the range of skills for which he has been qualified by training and previous employment.
